CGSL COVID-19 Digital Screener

As we continue to explore new ways to remain compliant with COVID-19 health regulations while keeping our members safe, we have made the decision to move to a digital contact tracking and screening format. 

Beginning July 24, 2021, prior to every game day played, we are asking (where possible) each player rostered and in attendance complete a Digital COVID-19 Self Assessment screening questionnaire. The Self Assessment Screening form can be found on the league's website as well as on a digital QR code on league signage at each diamond.

In the event you cannot complete the digital questionnaire the day of your game, a hard copy sign in sheet will be available with each Manager. However, to reduce unnecessary contact, it is strongly recommended that you utilize the digital option whenever possible. 

Managers will be responsible for ensuring that their rostered in attendance players have completed either the digital or paper questionnaire on game day. 

Member safety is our top priority, and as such, if you are not feeling well or have answered any of the questions YES and do not have a valid government exception, we ask that you refrain from attending and stay home.
